Instructions for Volunteer Physicians
Volunteer physicians must complete the CentroMed forms as part of the credentialing process. We are using the services of the Medical Society, and most of the credentialing process can be completed by using the services of the society. However, they cannot help us with the forms that are attached.
Again, these are CentroMed forms.
Delineation of Privileges form.
This is the form that the physicians must use to indicate the medical privileges they are requesting. In the column with the heading that says Privileges Requested, the physician can simply put a check-mark to identify the privileges requested. And it must be signed by the physician.
Physical Fitness Attestation.
Physicians will use this form to make a statement regarding their physical fitness.
The rest of the forms that are addressed to either the Bexar County or Bexar District, we will use to query the local courts downtown regarding charges or violations of criminal law.
Another absolute requirement for physicians is that they be CPR (basic life support).
